ASCLD/LAB-International Calibration Forms

The interactive forms listed below are provided for electronic completion. Any form for which See Accreditation Requirements Documents is indicated under the format column requires the completion and submission of a User License Agreement and completion and submission of the ISO 17025 Certification Statement before the form will be released by ASCLD/LAB.

The first step in obtaining all documents necessary to prepare and apply for ASCLD/LAB-International accreditation is to obtain a licensed version of ISO/IEC 17025:2005. The ISO document is copyrighted by ISO and therefore must be obtained from an authorized distributor. The document may be purchased directly from ISO at www.iso.org or from the American National Standards Institute (ANSI) at www.ansi.org. Other legal sources for ISO documents may be found by searching the internet. Outside the United States customers may also check with their applicable national standards publishing body.

Once you are in possession of a licensed version of ISO/IEC 17025:2005, you must submit a User License Agreement and an ISO Certification Statement to ASCLD/LAB to receive the corresponding ASCLD/LAB-International documents. If you represent a laboratory interested in preparing for ASCLD/LAB-International accreditation, all documents from ASCLD/LAB are provided to you at no cost. If you do not represent a laboratory there is a fee of $150.00 for the applicable ASCLD/LAB documents. At the sole discretion of ASCLD/LAB, and based upon intended use, complimentary copies of ASCLD/LAB documents may be provided for academic and research purposes.

At this time, many of the ASCLD/LAB-International forms are available only in Microsoft Word format as interactive (fill in) forms which may be opened and completed on your computer and printed. Simply download the form, open it on your computer, and type the required information in the gray-shaded fields. Use the TAB key to advance to the next field. It is generally advisable to save completed forms with a new file name specific to your laboratory.

Important Updates:


All ASCLD/LAB-International assessments conducted on and after April 1, 2011 will be to the 2011 supplemental requirements.
