Crime Laboratory Customers
The 2006 version of the ASCLD/LAB-International Supplemental Requirements, the 2006 ASCLD/LAB-International Field Assessment Guide, and the 2006 ASCLD/LAB-International Electronic Conformance File are available in electronic format only at no cost to crime laboratories interested in preparing for and seeking ASCLD/LAB-International accreditation. While the documents are free to crime laboratories, a User License Agreement and an ISO 17025:2005 Certification Statement must be completed and submitted to ASCLD/LAB before the documents will be released to you. To expedite receiving the documents, you may fax the license agreement form and certification statement to Caprice Fowler at (919) 773-2602 but the original (signed) license agreement and certification statement must still be mailed to ASCLD/LAB, attn: Caprice Fowler at 139 J Technology Drive, Garner, NC 27529.
Accreditation Consultants and other Non-Crime Laboratory Customers
The accreditation requirements documents are available in electronic format only to accreditation consultants and other non-crime laboratory individuals or entities for a fee of $150.00 per set (all three documents) or $50.00 per document. In addition to completing the order form below, a User License Agreement and an ISO 17025:2005 Certification Statement must be completed and submitted to ASCLD/LAB before the documents will be released to you.. For faster shipment of your order, you may fax the order form, payment by credit card form, user license agreement, and ISO17025:2005 certification statement to (919) 773-2602 but the original (signed) license agreement and certification statement must still be mailed to ASCLD/LAB.
The ISO/IEC 17025:2005 document is copyrighted by ISO and therefore must be obtained from an authorized distributor. The document may be purchased directly from ISO at www.iso.org or from the American National Standards Institute (ANSI) at www.ansi.org. Other legal sources for ISO documents may be found by searching the internet. Outside the United States customers may also check with their applicable national standards publishing body.
