Our forms are also available in Microsoft Excel and Word format as interactive (fill in) forms which can be opened and completed on your computer and printed. Simply download the form by either double clicking on the file link or right clicking on it. If you double click you will be asked whether to open or save the file. Select save and then select the directory where you want to store it and a name specific to your laboratory .
