How to Register

Please follow these steps to register

1. Download the Registration for Training Form.
   
   
2. Complete your personal and organization information.
   
   
3. Complete the payment information.
   
   
   • Following forms of payment are accepted:
     • Master Card
     • Visa
     • Check
     • Purchase Order
   • Checks and money orders must be made out to ASCLD/LAB.
     
     
4. Send registration form by one of the following methods.
